Job Description:
Description: The goals of Healthy Families Delaware Program are
to improve birth outcomes, reduce rates of child abuse, strengthen
families, enhance child health and developmental outcomes, and
promote family stability and economic self-sufficiency. Provide
assessment, prevention, and crisis intervention through home-based
and family focused support services to pregnant women and their
families following the Healthy Families America (HFA) evidence-base
home visiting best practice model. ESSENTIAL DUTIES AND
RESPONSIBILITIES
- Carry a case load of 17-20 families.
- Provide comprehensive home visiting services to women who are
pregnant or have newborns younger than three months of age,
following the Healthy Families America (HFA) evidence-base home
visiting best practice model.
- Provide home visitations that may include: pre and post-natal
education, parenting skills, child development education using
Partners for Healthy Babies curriculum and supplemental materials,
and early identification of learning disabilities, physical or
behavioral.
- Provide assessment, planning, referrals, advocacy, and
collateral contacts.
- Contact and engage new referrals to complete initial
eligibility screening assessment and Parent Survey tools.
- Assess physical, emotional, social and environmental needs of
women and their families as they relate to the program. Make the
appropriate referrals and follow-up to ensure services were
received.
- Review Parent Survey results with Supervisor and complete the
Service Plan.
- Adhere to HFA model time frame guidelines for completing
eligibility and Parent Survey.
- Complete all required HFA core and supplemental trainings
within specified time frames; some training may require out of
state travel.
- Develop and maintain community relationships, consult and
collaborate with other professionals involved in providing services
to women and families, and as requested promote public awareness of
the program.
- Prepare and maintain timely documentation and records in
accordance with agency, state accreditation, and other
requirements, policies, and procedures.
- Schedule quarterly joint home visits with the Supervisor.
- Attend and participate in weekly reflective supervision, team
and quarterly statewide meetings.
- Maintain agency standards regarding confidentiality, quality,
and productivity.
- Comply with applicable state, federal, agency and accreditation
requirements and standards.
- Serve on external and internal committees as assigned and/or
approved.
- Position travels primarily within New Castle County; however,
travel may be required to other counties depending on the referrals
and needs. EDUCATION and/or EXPERIENCE
- Associate degree in Early Childhood Education or related
discipline with two-three years of experience, required. Bachelor's
degree in Early Childhood Education or related field with
experience, preferred.
- Minimum of 3 to 5 years previous home visiting experience
working with children 0-3.
- Experience working with or providing services to children and
families, an ability to establish trusting relationships, and an
acceptance of individual differences, required.
- Experience and willingness to work with the culturally diverse
populations present among the site's population.
- Knowledge of infant and child development.
- Must be open to reflective practice (i.e. has capacity for
introspection, communicates awareness of self in relation to
others, recognizes value of supervision, etc.).
- Bilingual, desired.
- Must have valid driver's license, which meets the requirements
of the agency's insurance carrier and reliable
