third weekendGeneral SummaryLevel II Pharmacists provide prescribed
pharmaceuticals and drug information as needed for patient care
according to professional standards and practices. The Level II
Pharmacist position is a transitional role between the distributive
and clinical domains within the pharmacy department. Time spent in
each domain is contingent upon the needs of each facility and
department. Level II Pharmacists may advance to a Level III
Pharmacist position based upon job performance and successful
completion of the requirements for advancement established in the
Pharmacist Career Ladder Program (PCLP) criteria grids.Essential
Functions:Maintains the requirements located in the PCLP Mandatory
and Optional Criteria Grids for the Level I Pharmacist.Provides
oversight of safe, accurate, and timely medication order
verification including clarification of any medication orders with
discrepancies prior to dispensing.Provides pharmacy management and
oversight of: Integrity and security of medications in all care
locations Investigational drugs Preparation of injectable
chemotherapy Medication wastage, returns, and recalls
Reconciliation of controlled substance wastage Compounding of
formulations Automated dispensing cabinets Medication label
components and label qualityProvides medication therapy management
services in collaboration with other members of the health care
team including consultative services pursuant to the facilitys
available protocols which may include: Pharmacokinetic evaluation,
monitoring, and dosing (initiation and daily follow-up) Total
parenteral nutrition (TPN) consults Anticoagulation initiation and
follow-up pursuant to a consult Renal dose adjustments Intravenous
to oral conversions Additional clinical opportunities may be
available based upon departmental needs, clinical experience, and
performance, and may include unit-based pharmacist, antimicrobial
stewardship, patient care rounds, or pain management teamProvides
educational services to patients and healthcare providers regarding
medication therapy and/or medication safety concerns.Participates
in the training and education of pharmacy technicians, interns,
students, and/or pharmacy residents.Provides medication event
prevention, monitoring, and reporting with an increased emphasis on
patient safety.Utilizes technology to enhance patient safety by
ensuring the proper distribution, monitoring, and use of
medications. Participates in the assessment of
technology.Participates in quality, safety, and performance
initiatives related to medication use at the organizational and
departmental levels. Collaborates on operational system/process
research activities.Adheres to WellSpan Healths medication-use
policies and standard work. Develops and modifies polices and
standard work related to the use of medications within the health
system. Participates in the health systems formulary process,
ensuring an appropriate formulary system is in place.Common

development over the life span of the assigned patient population.
Assesses and interprets patient age specific data and provides
appropriate, age specific treatment for neonatal, pediatric, and
adult patients. Provides patient care for assigned patient age

by: complying with all applicable safety regulations; learning the
impact of medical errors and methodology that will lead to
reduction of errors; reporting actual and potential errors, as well
as hazardous conditions; identifying opportunities to standardize
processes and 'error proof' systems that will lead to increased
safety; and participating in safety education programs and root
cause analyses as required.Maintains established policies and
procedures, patient experience caring behaviors, objectives,
quality assessment, safety, environmental and infection control
standards.Maintains professional growth and development through
seminars, workshops, and professional affiliations to keep abreast
of latest trends in field of expertise.Provides outstanding service
to all customers; fosters teamwork; and practices fiscal
responsibility through improvement and innovation.Required for All

Pharmacy Bachelor's Degree RequiredWork Experience:Completion of a
PGY-1 residency. Required or3 years Hospital experience. Required
