Every 4th weekend, Employee must use their own vehicle Description / Comment : Job Summary: The Home Care Registered Nurse (RN) provides high-quality, patient-centered nursing care in a home setting. They assess patient needs, develop individualized care plans, administer treatments, and educate patients and families to promote independence and well-being. Key Responsibilities:
  • Conduct in-home assessments to evaluate patient health, environment, and care needs.
  • Develop and implement personalized care plans in collaboration with physicians and other healthcare providers.
  • Administer medications, wound care, IV therapy, and other medical treatments as prescribed.
  • Monitor patient progress, document care provided and adjust treatment plans as needed.
  • Educate patients and families on disease management, medication adherence, and self-care techniques.
  • Provide emotional support and guidance to patients and caregivers.
  • Coordinate with multidisciplinary teams, including therapists and social workers, to ensure comprehensive care.
  • Maintain accurate and timely patient records in compliance with HIPAA and healthcare regulations. Qualifications & Requirements:
    • Education: Associate's or bachelor's degree in nursing (BSN preferred).
