About IHP: IHP, a local physician-owned provider management organization, is expanding its services to new facilities and is looking to add a compassionate, caring, and talented Advanced Practice Provider (APP) to our team. We offer inpatient care in short-term and long-term hospitals, rehab facilities, post-acute care facilities, and skilled nursing facilities (SNFs).Basic Job Details:

• Position: Advanced Practice Provider (APP)
• Schedule: Monday to Friday, 8 AM - 5 PM or 9 AM - 6 PM
• Call Schedule: One in 3 weekends per month
• Patient Load: 14-16 patients per day
• Responsibilities: Provide medical care to patients in post-acute care and long-term facilities, utilize EMR for notesQualifications:
  • Board-certified APP, NP, or PA
  • 1 year of experience caring for the geriatric population and post-acute care is a plusBenefits:
