You will be responsible for conducting well-visits, managing chronic conditions, and collaborating with our multidisciplinary team to enhance the health and well-being of our homebound patients.Key Responsibilities:

• Conduct Well-Visits: Perform thorough physical examinations, health assessments, and routine screenings for elderly patients during home visits.
• Patient Management: Develop, implement, and evaluate individualized care plans. Monitor chronic conditions, medication regimens, and provide education on disease management.
• Collaboration: Work closely with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated and comprehensive care.
• Documentation: Maintain accurate and timely electronic medical records, including patient history, assessment findings, and care plans.
• Patient Education: Provide education to patients and their families on health maintenance, disease prevention, and managing chronic conditions.
• Follow-Up Care: Coordinate and manage follow-up visits, laboratory tests, and referrals to specialists as needed.
• Emergency Response: Identify and respond to medical emergencies during home visits, providing initial care and coordinating further treatment as necessary.
• Quality Improvement: Participate in quality improvement initiatives, contributing to the development and implementation of best practices in home care.Requirements:
  • Master's degree in Nursing from an accredited Nurse Practitioner program.
  • Licensure: Current and valid Nurse Practitioner license in NJ. Certification in Adult-Gerontology Primary Care or Family Nurse Practitioner preferred.
  • Experience: Minimum of 1 year of experience in geriatrics or home care settings.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal abilities.
  • Compassionate approach with a focus on patient-centered care. Please note- The annual base salary range is $140k to $155k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
