  • Social Worker/Discharge Planner - 40 hrs Days
  • Shifts: 8am - 4:30pm Day and rotating weekends
  • START DATE: 10/14
  • Will accept RN Discharge Planner or Licensed Social Worker
  • Social worker must be eligible for LMSW/LGSW/LCSW - must have taken and passed the exam already
  • Work Setting skills Req: Cerner, Acute Hospital, Insurance Company, Long Term Acute Care/Rehab/Skilled Nursing
  • Work Setting skills Preferred: Home Health, Hospice, telephonic, previous charge experience
  • Case Management/Utilization Review skills required: Admission Criteria, Benefits Eligibility, Care coordination, Discharge Planning, Needs Assessment/ Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker's comp case management
  • Regulatory Skills Required: CMS: Centers for Medicare and Medicaid Services, Department of Health, HIPAA (The Health Insurance Portability and Accountability Act), The Joint Commission/ Core Measure/National Safety Goals
  • Regulatory Skills Preferred: Worker's Compensation
  • Any Special Requirements: Must have experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters
  • Patient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU
  • Cerner is required
  • Uniform: Business Casual-Associate's appearance should be clean, neat and professional. NO SCRUBS
  • 2 Years Minimum Experience Required
  • Must have 1 prior travel assignment
