Job Description:
A company is looking for a Case Management Liaison - Remote -
PI-Population Health, FT Days.
Key Responsibilities
Assist with compiling resource information and developing resource
files
Communicate utilization review and discharge planning information
to internal and external customers
Gather clinical, psychosocial, and financial information, referring
patients to professional staff as needed
Required Qualifications, Training, and Education
High School Diploma or GED equivalent
Three (3) years of progressively responsible and directly related
work experience
Knowledge of medical terminology
Knowledge of Windows-based office software, computers, and
operating systems
